Richard Frost Youth Memorial Scholarship (RFYMS) recipients must show proof that you completed the event or course you were awarded to attend. Examples of proof of completion include certificates of completion, final grades, a letter from the organization facilitating the training, a video with a narration of the recipient at the event, or this form. Supporting photos are encouraged. PLEASE DON’T SUBMIT THIS FORM UNTIL YOUR EVENT OR COURSE IS COMPLETED.
